Service Agreement for Group Therapy Patients

PARTIES

This Service Agreement is for your child (The Client), and is made between yourself (The Parent / Guardian) and Believe and Achieve Paediatric Therapy.

SUMMARY OF SCHEDULED SUPPORTS

This Service Agreement is made for the purpose of providing therapy services under the participant’s NDIS plan. Believe and Achieve Paediatric Therapy agrees to supply therapeutic intervention consisting of the following services:

  • Therapy Intervention
    • Face-to-face services in a group setting
    • Parent / educator coaching as required
    • Non face-to-face services
    • Reports and letters of recommendation as required / requested

RESPONSIBILITIES

Provider's Responsibilities 

Believe and Achieve Paediatric Therapy agrees to: 

  • Review the provision of therapy services with The Parent / Guardian regularly
  • Communicate openly and honestly in a timely manner
  • Treat the participant with courtesy and respect
  • Protect the participant’s privacy and confidential information
  • Keep accurate records on the supports provided to the participant
  • Provide support in a manner consistent with all relevant laws, including the National Disability Insurance Scheme Act 2013 and rules, and the Australian Consumer Law
  • Review the service with the participant at the conclusion of The Client’s NDIS plans and provide a progress report on achievements towards goals to The Parent / Guardian and the NDIS as required by the NDIA


Parent / Carer of Participant's Responsibilities 

I, the Parent / Guardian, agree to: 

  • Adhere to allotted time slot for therapy
  • Provide and receive verbal feedback / updates during the allotted face-to-face time
  • Talk to Believe and Achieve Paediatric Therapy if I have any concerns about the services or the supports being provided
  • Inform the provider immediately if the participant’s NDIS plan is suspended or replaced by a new NDIS plan or the participant stops being a participant in the NDIS
  • Communicate openly and honestly in a timely manner
  • Give Believe and Achieve Paediatric Therapy reasonable notice (see Cancellation Policy and Changes to the Agreement) should I need to change any arrangement so that appropriate adjustments, if necessary, can be made
  • Give Believe and Achieve Paediatric Therapy reasonable notice (see Cancellation Policy and Changes to the Agreement) should I wish to cease this agreement

CANCELLATION POLICY AND CHANGES TO THIS AGREEMENT

Should my child not be able to attend a booked session, I will notify Believe and Achieve Paediatric Therapy as soon as possible.


Late arrivals will be charged the full fee for the scheduled appointment. Late arrivals will result in a shorter consultation and the session will conclude at the allocated concluding time.


For supports delivered to a group of participants, if a participant cancels their attendance and if the provider is unable find another participant to attend the group session in their place then, if the other requirements for a short notice cancellation are met, the provider is permitted to bill the participant who has made the short notice cancellation at the previously agreed rate that they would have billed if the participant had attended the group. All other participants in the group should also be billed as though all participants had attended the group. 


Due to the nature of group therapy, cost is per term.  As a result, the full fee will be charged for any missed sessions within the term, including cancellations and no-shows.

BILLING AND PAYMENTS

Believe and Achieve Paediatric Therapy will seek payment for supports provided on a regular basis. Prices are subject to changes to the NDIS Pricing Arrangements and Price Limits. Written notice will be provided prior to service price changes being billed.


Plan managed participants: The Parent / Guardian has nominated the Plan Management Provider to manage the funding for NDIS services provided under this Service Agreement. Invoices will be issued to the Plan Manager.


OR


Self managed participants: The Parent / Guardian has chosen to self-manage the funding for NDIS services provided under this Service Agreement. After providing those services, the provider will generate an invoice on conclusion of service provision for the participant to pay. Payment is due within 5 days from the date of invoice. Late payments will result in future appointments being postponed until payment is received.

FEEDBACK / COMPLAINTS AND DISPUTES

Believe and Achieve Paediatric Therapy welcomes feedback on its service provision.

If you have a concern or complaint, please refer to the complaints procedure which has been provided to you, and contact Ruby Challita in the first instance if appropriate.


If the complaint/dispute remains unresolved and is of serious nature, you can contact the NSW Ombudsman by calling 02 9286 1000 or 1800 451 524. Complaints can also be emailed to nswombo@ombo.nsw.gov.au and a complaint form can be found in the NSW Ombudsman Website www.ombudsman.gov.au.

PRIVACY POLICY

Believe and Achieve Paediatric Therapy will create a confidential patient file to document all necessary information throughout the period of intervention. 


All personal information will be stored on a secure digital medical database on Halaxy and physical files are stored in a secure, locked filing system.
