This Service Agreement is for your child (The Client), and is made between yourself (The Parent / Guardian) and Believe and Achieve Paediatric Therapy.
This Service Agreement is made for the purpose of providing therapy services. Believe and Achieve Paediatric Therapy agrees to supply therapeutic intervention consisting of the following services:

Should I and/or my child not be able to attend a booked session, I will notify Believe and Achieve Paediatric Therapy as soon as possible.
Late arrivals will be charged the full fee for the scheduled appointment. Late arrivals will result in a shorter consultation and the session will conclude at the allocated concluding time.
Cancellations made within 7 days of scheduled services will result in a charge of 100% of the agreed amount for therapy sessions being charged. Failure to attend consultations without sufficient notice will result in a charge of 100% of the agreed amount for therapy sessions being charged.
Therapy spaces are reserved for your child at the agreed upon frequency (weekly or fortnightly). Believe and Achieve Paediatric Therapy is unable to hold a therapy space for children who are not receiving therapy services consistently and regularly.
Believe and Achieve Paediatric Therapy has an 80% attendance requirement during school terms (excluding school holidays), applicable to planned absences only. A planned absence is a cancellation with more than 7 days notice, therefore the 80% attendance requirement does not include late cancellations due to illness or other unforeseen reasons.
For children who attend therapy weekly, Believe and Achieve Paediatric Therapy is unable to hold a therapy space if there will be more than 2 planned absences in a school term.
For children who attend therapy fortnightly, Believe and Achieve Paediatric Therapy is unable to hold a therapy space if there will be more than 1 planned absence in a school term.
If a family is planning to have more than the allocated planned absences in a term, families may request alternate services in their child's allocated therapy time slot to retain their current therapy space. Alternate services may include, but are not limited to resource making, reports, emailing teachers / educators, parent education or home programming.
Alternatively, families may opt to have a break from therapies to allow for another child to benefit from this therapy time slot.

If your child’s regular therapist has to cancel a session due to illness or scheduled leave, Believe and Achieve Paediatric Therapy will make every effort to schedule an alternative therapist in the same appointment slot so your child is not missing out on the therapy they require. The alternate therapist will follow a session plan written by your regular therapist.
Should either party need to substantially change when or how supports are provided, I agree to give two (2) weeks’ notice during service operating hours.
Should either party require this Agreement to end, I agree to give two (2) weeks written notice during the service’s operating hours. Any cancellations made during this notice period will be billed accordingly.
Believe and Achieve Paediatric Therapy will seek payment for supports provided on a regular basis. Prices are subject to changes to the NDIA Pricing Arrangements. Written notice will be provided prior to service price changes being billed.

Sessions that run over the allotted time will be billed accordingly.
Travel for mobile service provision is billable. Travel costs are calculated in 5 minute increments, and are billed by calculating the sum of time traveled to and from the therapy location of choice, for a maximum of thirty (30) minutes according to the corresponding line item in the current NDIA Pricing Arrangements.
After providing those services, the provider will generate an invoice on conclusion of service provision for the participant to pay. Payment is due within 7 days from the date of invoice.
Late payments will result in future appointments being postponed until payment is received.
Believe and Achieve Paediatric Therapy welcomes feedback on its service provision.

If you have a concern or complaint, please refer to the complaints procedure which has been provided to you, and contact Ruby Challita in the first instance if appropriate.
If the complaint/dispute remains unresolved and is of serious nature, you can contact the NSW Ombudsman by calling 02 9286 1000 or 1800 451 524. Complaints can also be emailed to nswombo@ombo.nsw.gov.au and a complaint form can be found in the NSW Ombudsman Website www.ombudsman.gov.au.
Believe and Achieve Paediatric Therapy will create a confidential patient file to document all necessary information throughout the period of intervention.
All personal information will be stored on a secure digital database on Halaxy and physical files are stored in a secure, locked filing system.
